easy to make decisions about your health and health care. What questions
should you ask your doctor? What are the treatment options? How
can you reduce your risks or prevent long-term complications?
Whether you
just want some basic advice, or detailed information about a serious
illness, health coaches can help answer your questions, discuss
your options and empower you to make informed decisions. Think of
them as your partners in health and well-being.
About LifeSteps health coaches
Health coaches
are specially trained health professionals, including nurses, dietitians
and respiratory therapists, with extensive health care experience.
They're available 24 hours a day, 7 days a week to assist you and
your family.
When should you contact a health coach?
Contact a LifeSteps
health coach any time you have a health-related question. Here are
a few examples:
- Your doctor
has recommended a test and you want to learn more about it and
prepare for it.
- You want
to discuss a specific condition and your treatment choices.
- You want
to talk to your doctor about a health matter and need help preparing
the right questions to ask.
- You want
information about fitness and other healthy habits.
Health coaches
won't replace your doctor or other health care providers. Instead,
they'll help you get the most from every visit to your doctor.
Who can call
a health coach?
LifeSteps health
coaches are available to U.S. employees and their family members
who are eligible for coverage under our health insurance plans,
regardless of whether you or your eligible family members currently
participate in one of American Standard's health insurance plans.
Your confidentiality is protected
LifeSteps health
coaching is a service provided by Health Dialog®, a third-party
provider of services to the company's group health benefit plans,
as part of the LifeSteps program for American Standard Companies
employees and their families. To address the specific needs of employees
and their families, Health Dialog will have access to personal health
information directly from insurance providers. Any personal health
information provided to a health coach is protected by federal and
state privacy laws and will not be disclosed to American Standard
except under limited circumstances as authorized by law for administration
of the company's group health plans.
What is Health
Dialog?
Health Dialog
is a third-party provider that works in collaboration with the not-for-profit
Foundation for Informed Medical Decision Making. The Foundation
was established by top physicians from Dartmouth and Harvard medical
schools to improve the quality of medical decisions by helping people
communicate more effectively with their health care providers. The
Foundation works with Health Dialog to develop programs and educational
materials based on the latest medical research to support quality
health care decision-making.
