Next Life "Step": Identifying Our Health
Risks
A new interactive
LifeSteps tool for all U.S. employees is launching Monday, Nov.
15: the personal health assessment. It will help you identify and
understand any health risks you may face, so you will be better
able to make lifestyle changes to improve your well-being.
Starting Monday,
Nov. 15, you'll find the personal health assessment tool in the
LifeSteps Online section of Employee Services on MyStandard. It
will also be available to you from home or anywhere you have Internet
access through www.asdlifesteps.com.
The assessment will take about 15 minutes to complete - and if you
complete yours early, you'll have the chance to win a great raffle
prize.
The personal
health assessment will be available to all U.S. employees and their
adult dependents (a spouse or eligible domestic partner)
If
you complete your personal health assessment by Dec. 15, 2004, you
can enroll in a raffle for these prizes:
- American
Express gift checks valued at $100 to $500
- $50 gift
certificates to Barnes&Noble.com, one of our E-Mall vendors
The raffle is
also open to spouses or eligible domestic partners. To increase
your chances of winning a raffle prize, encourage your spouse or
eligible domestic partner to complete their own assessment and enter
the raffle by Dec. 15. Their raffle entry means one more chance
for you to win! (Please note: Spouses and eligible domestic partners
are not eligible to win a raffle prize themselves. But by
completing their own assessment and entering the raffle, they'll
get you another chance to win a prize.)
The personal
health assessment asks various questions about your health, such
as cholesterol and blood pressure, and lifestyle habits, such as
how frequently you exercise. The answers to these questions will
help identify your health risks.
Once you complete
the assessment, you'll receive a confidential, personalized
report summarizing the results based on the responses you provided.
You can use this information to reduce your health risks by working
with a LifeSteps health
coach. Your health coach can help you address your health risks
before they become serious problems.
Look for a packet
in the mail at home with all the details on the personal health
assessment. Don't delay, complete it today and get in on
the raffles! You can also look for more LifeSteps educational resources
and opportunities in the coming months, including incentives when
you and your family complete certain educational programs or commit
to healthy and safe practices.

Keep in mind that personal health assessment results, as well as
any personal information you or your family members provide, are
protected by federal and state privacy laws.
