Learning about your health risks by completing the LifeSteps personal health assessment can help you focus your efforts on the behaviors that can make the greatest difference to your overall well-being. The personal health assessment is available to all U.S. employees and their adult dependents (a spouse or eligible same-sex domestic partner).

If you didn't win this time, you're not out of luck. Complete your personal health assessment by Feb. 4 to enroll in the second raffle drawing! You'll have the chance to win one of five American Express gift checks valued from $100 to $500, or one of two Dell computers gift certificates valued at $250. The assessment will take about 15 minutes to complete. If you completed the personal health assessment, entered the first raffle, but didn't win, you're automatically entered into the later raffle drawings.

Only employees are eligible for the raffle prize, but you can double your chances of winning by having your adult dependent, such as a spouse, also complete a personal health assessment by Feb. 4. Your adult dependent's entry will count as an extra raffle entry on your behalf.

How to assess your personal health
You can complete your personal health assessment at any time. Simply log on to LifeSteps Online through the Employee Services area of MyStandard from work, or www.asdlifesteps.com from home or anywhere you have Internet access. Once you complete your assessment, you'll have the option to enter the raffle.

The personal health assessment is a confidential tool that calculates your health risks based on your responses to a series of questions about your health and lifestyle habits. You'll receive a personalized report with your results after you complete the assessment. By identifying your health risks, you and your family can make lifestyle changes to reduce your health risks and get more out of the LifeSteps resources that are available.
